I. Conditions for two-way referrals
(i) Conditions for hospital referral:
1. Cases of acute and critical illnesses in various clinical departments, which are difficult to implement effective treatment.
2. Difficult and complicated cases that cannot be diagnosed.
3. Cases in mass casualty events with limited disposal capacity.
4. Cases that need to be further examined and diagnosed at a higher level of medical institutions.
5. Other cases that cannot be disposed of due to technical and equipment limitations.
(B) Hospital transfer conditions:
1. Cases whose condition is stabilized after acute treatment and need to continue rehabilitation treatment.
2. Cases with clear diagnosis that do not need special treatment.
3. advanced non-surgical treatment and hospice care for patients with various malignant tumors.
4. Cases of chronic diseases requiring long-term treatment.
5. Elderly care cases.
6. Cases of general common and frequent diseases.
II. Referral procedures
(I) Referred patients
The hospital is responsible for receiving patients referred by municipal and county hospitals, township health centers and community health centers, as well as patients in stable condition transferred back from higher-level hospitals.
After receiving the referred patients, the hospital will record the referrals in the medical records and give priority to the patients in consultation, examination, payment and medication; those who need to be hospitalized will be given priority in the arrangement.
(II) Transferring out patients
1、According to the condition of patients who need to be transferred to a higher hospital for further treatment, after obtaining the consent of the department director and the consent of the patient and his/her family members, the patient shall record the referral in the medical record and provide relevant information about his/her condition; if the condition requires, the medical staff shall escort the patient to the hospital to ensure that he/she is safely transferred to a higher hospital and to do a good job of handing over the patient's condition.
2. For those who meet the conditions for downward referral, after obtaining the consent of the department director and the consent of the patient and his/her family members, the referral will be recorded in the medical record, and relevant information on the patient's condition will be provided. The patient's family members will transfer the patient to the lower level hospital with relevant diagnostic and treatment information.
